Loom of the Black Mist

The Fortress of the Black Mist was never the source. Under it, down a stairwell the fortress's own builders bricked over, there is a loom. It is enormous, older than the fortress, and it is strung with breath: every person who has ever suffocated in the mist exhaled one last time, and that exhalation was caught, carded, spun into thread and woven. The mist rolling over the island is not weather. It is fabric, and the fortress was built to keep it from unrolling all at once.
The weavers are the dead who worked the loom in life and did not notice they had stopped being alive. They are courteous. They will pause, and apologise, and then resume. That is the register of the whole dungeon and the reason it unsettles more than the fortress above it: nothing here is angry with you, and nothing here is going to stop. It is a textile mill that never got to close, and the horror is the routine.

The final boss
Vaelith, the Mistmother is not a monster in the usual sense. She is the loom's operator, has been for nine hundred years, and she is very tired. She would like to be relieved. The Keeper of the Last Breath stands between you and her, which she did not ask for. When she tests you, she is testing whether you can hold the shuttle, and she would be glad to be wrong about the answer.
Before you enter
The recommended level is 45. Treat it as a starting point: farm earlier tiers, upgrade gear, and adjust your pet when a boss stops you.
Difficulty inside a dungeon comes from one number that climbs with the tier, and the step between tiers is a large fraction of the gap between dungeons. So a wall at tier 7 here is not a signal to grind this dungeon harder - it usually means the opening tiers of the next dungeon are a better use of the hour, and you come back with the gear they dropped.
